We currently have a survey open to understand the lived experience of people with long term conditions and disabilities. The survey will not take long to complete and everything you tell us is anonymous and confidential. The survey is open until midnight on 30.04.25. As a thank you for taking part in the survey, there will be a random prize draw for 5 winners to receive a £40 gift bag, helping them to be active at home.

Full terms & conditions

All Move In Erewash (AMIE) Prize Draw Terms and Conditions 

  1. The prize draw is open to all Erewash residents aged 18 years or over who have a long-term health condition, illness or disability, except employees of the Promoter, their families, agents or any third party directly associated with administration of the prize draw.
  2. The prize draw is free to enter. You may enter the prize draw by validly completing the AMIE survey and selecting “Yes” to question 13 and entering your valid email address in question 14 of the AMIE survey. The AMIE survey is available at:Let's Talk Erewash.
  3. All entries must be submitted via completion of the AMIE survey as set out in condition 2 above and only one entry per person is permitted.
  4. The opening date for entries is 12:00 on 03 March 2025. The closing date of the prize draw is 23:59pm on 30 April 2025. Entries received after this time may not be valid.
  5. The Promoter accepts no responsibility for entries not successfully completed due to a technical fault, technical malfunction, computer hardware or software failure, satellite, network or server failure of any kind.
  6. Five (5) winners will be chosen by random draw performed by a computer process on 15 May 2025 ("Draw Date").
  7. The five (5) winners will each receive one (1) gift bag worth £40.00 (forty pounds) (the gift bags shall contain item(s) that aid being active at home but the Promoter shall choose the exact contents at their sole discretion).
  8. The winners will be notified by email or telephone (using details provided at entry) before 30 May 2025 and must either provide a postal address or agree to collect the prize in person from Erewash Museum, High Street, Ilkeston DE7 5JA to claim their prize. If a winner does not respond to the Promoter within 7 days of being notified by the Promoter, then the winner's prize will be forfeited and the Promoter will be entitled to select another winner in accordance with the process described above.
  9. The prizes will be sent to the winners by post or collected from Erewash Museum, High Street, Ilkeston DE7 5JA at a pre-agreed date and time.
  10. The prizes for the winners is non-exchangeable, non-transferable and no cash alternative is offered.
  11. The decision of the Promoter regarding any aspect of the prize draw is final and binding and no correspondence will be entered into about it.
  12. The Promoter must either publish or make available information that indicates that a valid award took place. To comply with this obligation the Promoter will send the surname and town of major prize winners and, if applicable copies of their winning entries, to anyone who emails This email address is being protected from spambots. You need JavaScript enabled to view it.  or writes to Community Health & Wellbeing Development, Erewash Museum, High Street, Ilkeston DE7 5JA (enclosing a self-addressed envelope) within one month after the closing date stated in condition 4. If you object to any or all of your surname, town and winning entry being published or made available, please contact the Promoter via email at: This email address is being protected from spambots. You need JavaScript enabled to view it. or via telephone on 0115 850 8333.  In such circumstances, the Promoter must still provide the information and winning entry to the Advertising Standards Authority on request.
  13. Participants are deemed to have accepted and agreed to be bound by these terms and conditions upon entry. The Promoter reserves the right to refuse entry, or refuse to award the prize to anyone in breach of these terms and conditions.
  14. The Promoter reserves the right to hold void, cancel, suspend, or amend the promotion where it becomes necessary to do so.
  15. Insofar as is permitted by law, the Promoter, its agents or distributors will not in any circumstances be responsible or liable to compensate the winner or accept any liability for any loss, damage, personal injury or death occurring as a result of taking up the prize except where it is caused by the negligence of the Promoter, its agents or distributors or that of their employees. Your statutory rights are not affected.
  16. Personal data supplied during the course of this promotion will only be processed as set out in the Promoter's privacy policy available at: privacy page . See also condition 12 with regard to the announcement of winners.
  17. These terms and conditions and this promotion are governed by English law. If any entrants to this promotion wish to take court proceedings, then they must do this within the courts in England and Wales
